25.2px; background-color: rgb(255, 255, 255);\">　　1．1糖皮质激素和免疫抑制剂</span><br/><span style=\"font-family: Arial, 宋体; font-size: 14px; line-height: 25.2px; background-color: rgb(255, 255, 255);\">　　糖皮质激素可抑制炎症和免疫过程，减轻肺泡炎，从而延缓PF的进程。目前研究资料显示10％～30％的特发性肺纤维化(1PF)患者对糖皮质激素有一定疗效，但缺乏大样本、双盲、随机、对照的临床试验来评价其效果。且越来越多的研究认为，激素治疗者中少部分可出现短暂的临床反应，但并不能延长其生存期。欧美国家推荐糖皮质激素和免疫抑制剂联合应用治疗IPF。免疫抑制剂有抑制炎症和免疫反应的作用。但目前尚无确凿证据表明这种联合治疗方法有很好的治疗价值。</span><br/><span style=\"font-family: Arial, 宋体; font-size: 14px; line-height: 25.2px; background-color: rgb(255, 255, 255);\">　　1．2大环内酯类抗生素</span><br/><span style=\"font-family: Arial, 宋体; font-size: 14px; line-height: 25.2px; background-color: rgb(255, 255, 255);\">　　大环内酯类抗生素有抗炎和免疫调节作用。Desaki等用红霉素治疗博莱霉素(BLU)诱导的PF模型，发现该药抑制了肺泡巨噬细胞(AH)的核因子-K B(NF-K B)活性，抑制了肺组织白细胞介素-1β(IL-1β)和转化生长因子-β(TGF-β)mRNA表达，减轻了肺泡炎和纤维化。EM703的治疗作用机制可能是通过抗炎和调节肺纤维母细胞TGF-β的信号转导。阿奇霉素可阻断NF-K B与肿瘤坏死因子-α(TNF-α)、TGF-β形成的恶性循环，抑制巨噬细胞分泌胶原酶。李立等[1]研究发现阿奇霉素治疗IPF患者具有与口服泼尼松相当的疗效，且副作用少。</span><br/><span style=\"font-family: Arial, 宋体; font-size: 14px; line-height: 25.2px; background-color: rgb(255, 255, 255);\">　　2抗纤维化治疗</span><br/><span style=\"font-family: Arial, 宋体; font-size: 14px; line-height: 25.2px; background-color: rgb(255, 255, 255);\">　　2．1秋水仙碱</span><br/><span style=\"font-family: Arial, 宋体; font-size: 14px; line-height: 25.2px; background-color: rgb(255, 255, 255);\">　　秋水仙碱可抑制FB增殖、趋化、迁移和胶原沉积，减少PF患者AM释放FB因子和纤维连接素。许多体外实验研究显示此药可减缓纤维化进程。但一组大样本回顾性研究显示，用秋水仙碱治疗的IPF患者与用激素或未接受治疗者相比，在减缓肺功能下降及延长生存期方面无明显差异。该药治疗的有效性仍需大量的临床研究证实。</span><br/><span style=\"font-family: Arial, 宋体; font-size: 14px; line-height: 25.2px; background-color: rgb(255, 255, 255);\">　　2．2吡啡尼酮</span><br/><span style=\"font-family: Arial, 宋体; font-size: 14px; line-height: 25.2px; background-color: rgb(255, 255, 255);\">　　吡啡尼酮在体外试验中能减少TGF-β和血小板衍生生长因子(PDOF)等的表达，从而抑制纤维化。在体内试验中发现其能抑制TGF-β引起的胶原合成，减少ECM的产生，阻断PDGF的促有丝分裂作用，抑制FB的增生。Azuma等在研究中发现，IPF患者经口服吡啡尼酮治疗，其肺功能及呼吸困难症状均较安慰剂对照组有所改善。但该药的有效性、安全性还需进一步证实。</span><br/><span style=\"font-family: Arial, 宋体; font-size: 14px; line-height: 25.2px; background-color: rgb(255, 255, 255);\">　　2．3血管紧张素转换酶抑制剂(ACEI)／血管紧张素Ⅱ的Ⅰ型受体(AT1)拮抗剂</span><br/><span style=\"font-family: Arial, 宋体; font-size: 14px; line-height: 25.2px; background-color: rgb(255, 255, 255);\">　　正常肺组织中存在局部内源性肾素—血管紧张素系统，其分泌的血管紧张素在其转换酶的作用下生成血管紧张素Ⅱ(AngⅡ)。AngⅡ能直接刺激FB生长及胶原分泌，诱导内皮细胞及上皮细胞凋亡，促进PF的发生。ACEI和ATl拮抗剂可阻断AngⅡ的致纤维化作用。何平平等[2]的研究显示，应用卡托普利和氯沙坦治疗BLU诱导的PF大鼠，均可抑制PF。3抗氧化剂</span><br/><span style=\"font-family: Arial, 宋体; font-size: 14px; line-height: 25.2px; background-color: rgb(255, 255, 255);\">　　3．1N-乙酰半胱氨酸CNAC)</span><br/><span style=\"font-family: Arial, 宋体; font-size: 14px; line-height: 25.2px; background-color: rgb(255, 255, 255);\">　　NAC是谷胱甘肽(GSH)的前体，GSH．是细胞内重要的抗氧化剂。NAC／GSH能减少FB增生和削弱细胞内基质形成，还可抑制前炎性和前纤维化的信号转导和细胞因子的产生。有研究发现，IPF患者经大剂量NAC治疗其肺功能得到明显改善。Zeki Yildirim等证实NA可明显减轻BLM诱导的PF小鼠的PF。</span><br/><span style=\"font-family: Arial, 宋体; font-size: 14px; line-height: 25.2px; background-color: rgb(255, 255, 255);\">　　3．2其它药物</span><br/><span 255);\">　　如氨溴索、烟酸、牛磺酸等也在研究当中。有研究发，现早期应用氨溴索，可清除氧自由基，减轻细胞损伤，并能减少TNF-α生成或释放，干预纤维化形成。牛磺酸和烟酸减轻PF的可能机制是通过下调TNF-α等细胞因子表达，减少胶原生成。</span><br/><span style=\"font-family: Arial, 宋体; font-size: 14px; line-height: 25.2px; background-color: rgb(255, 255, 255);\">　　4抗凝剂</span><br/><span style=\"font-family: Arial, 宋体; font-size: 14px; line-height: 25.2px; background-color: rgb(255, 255, 255);\">　　有证据表明：PF患者肺泡灌洗液中凝血酶水平升高，而凝血酶除了在凝血中有作用外，也可通过诱导PDGF直接刺激肺FB和平滑肌细胞原胶原的产生。因此，抗凝剂在理论上可以抗纤维化。Kubo等在研究中比较了强的松治疗组与华法令或低分子肝素与强的松联合治疗组的效果后得出，抗凝治疗可减少急性加重的IPF患者的死亡率。但仍缺乏大样本、双盲、随机实验以证实其作用及作用机制。</span><br/><span 255);\">　　TGF-β是目前研究最深入、最重要的致纤维化的细胞因子，它参与FB的分化及增殖，刺激或抑制多种细胞因子的合成与分泌，从而促进细胞间质的形成。用SD-208(TGF-β受体抑制剂)治疗PF小鼠，可抑制纤维化的形成和发展。Smad7(TGF-β信号转导拮抗剂)可抑制BLM诱导的PF小鼠的Ⅰ型前胶原和其他前纤维化分子的表达。修饰素(Decorin)是TGF的天然抑制剂，还可降低肺羟脯氨酸。但TGF-β的生物作用广泛，阻断它的活性同时，不可避免地影响其正常的生理功能，很可能导致致命性的类自身免疫性疾病。</span><br/><span line-height: 25.2px; background-color: rgb(255, 255, 255);\">　　IFN-γ具有多种生物学效应：一方面，它可抑制FB增殖，并促进其凋亡，减少胶原合成：另一方面，它又可促进淋巴细胞趋化，单核细胞增殖活化；继而分泌多种促增殖因子，间接促进细胞增殖和组织纤维化。Raghu等在IFN-γ 1b治疗IPF的研究中发现IFN-γ 1b组呼吸困难发生率、氧气使用率较对照组有减少趋势。Karla等证实IFN-γ对于终末期IPF的治疗无效。目前有关IFN的实验与临床研究仍在深入。</span><br/><span style=\"font-family: Arial, 宋体; font-size: 14px; line-height: 25.2px; background-color: rgb(255, 255,
