255);\">　　脑出血，俗称脑溢血，属于“脑中风”的一种，是中老年高血压患者一种常见的严重脑部并发症。脑出血是指非外伤性脑实质内血管破裂引起的出血，最常见的病因是高血压、脑动脉硬化、颅内血管畸形等，常因用力、情绪激动等因素诱发，故大多在活动中突然发病，临床上脑出血发病十分迅速，主要表现为意识障碍、肢体偏瘫、失语等神经系统的损害。它起病急骤、病情凶险、死亡率非常高，是目前中老年人致死性疾病之一。</span><br/><span style=\"font-family: Arial, 宋体; font-size: 14px; line-height: 25.2px; background-color: rgb(255, 255, 255);\">　　１　 资料与方法</span><br/><span style=\"font-family: Arial, 宋体; font-size: 14px; line-height: 25.2px; background-color: rgb(255, 255, 255);\">　　１．１　 一般资料。患者80例，均为我院收治的脑出血患者，所有病例均行头颅ＣＴ检查明确出血部位，并排除严重的心、肝、肾等疾病或功能衰竭、颅内及全身感染和凝血机制障碍的患者，其中男5２例，女性2８例，年龄５０～７８岁，平均年龄５４．５±６．５岁。出血部位：颞部35例，额部22例，颞顶部13例，枕部10例；发病时间３～７２ｈ，出血量在３５～６０ｍｌ。入院时格拉斯哥昏迷评分（ＧＣＳ）评分：３分～８分4５例，９分～１２分3５例。</span><br/><span style=\"font-family: Arial, 宋体; font-size: 14px; line-height: 25.2px; background-color: rgb(255, 255, 255);\">　　大部分患者存在偏瘫或意识障碍，未发生脑疝。临床表现包括头痛、呕吐、意识障碍、偏瘫、失语等。经急救治疗和护理后，72例病人症状有所改善，其中１２例病人进行开颅手术，68例病人未进行开颅术。</span><br/><span style=\"font-family: Arial, 宋体; font-size: 14px; line-height: 25.2px; background-color: rgb(255, 255, 255);\">　　1. 2 急救护理</span><br/><span style=\"font-family: Arial, 宋体; font-size: 14px; line-height: 25.2px; background-color: rgb(255, 255, 255);\">　　2．1　 绝对卧床休息，一旦确诊为脑出血，应尽量减少搬动，以就地抢救为好，如果病人躁动可适当使用镇静剂，剂量不可过大，以免影响观察意识状态。</span><br/><span style=\"font-family: Arial, 宋体; font-size: 14px; line-height: 25.2px; background-color: rgb(255, 255, 255);\">　　2．2 　体位。为减少脑血流量，降低颅内压，病人头部置一软枕，约抬高１５～３０度左右，并偏向一侧，口稍向下，以利口水及呼吸道分泌物自然引流，切忌无枕仰天平卧。</span><br/><span style=\"font-family: Arial, 宋体; font-size: 14px; line-height: 25.2px; background-color: rgb(255, 255, 255);\">　　2．3　 保持呼吸道通畅，病人常有喉肌松弛，舌根后坠或因大量分泌物流入气管而堵塞呼吸道，可用通气管或用舌钳将舌头向外拉，以保持呼吸道通畅，勤吸痰，必要时气管切开，行气管插管，使用人工呼吸机，以避免缺氧引起的进一步脑组织损害。</span><br/><span style=\"font-family: Arial, 宋体; font-size: 14px; line-height: 25.2px; background-color: rgb(255, 255, 255);\">　　2．4 　饮食。起病７２小时禁食，三天后待病情稳定放置胃管，给予低脂高蛋白的流质和一定量水分，若有消化道出血，不能进食，应常抽取咖啡色胃内容物抽空气，可注入氧化铝凝胶，以保护胃粘膜或注入止血药，甲氰咪胍或肾上腺素溶液（１％）。</span><br/><span style=\"font-family: Arial, 宋体; font-size: 14px; line-height: 25.2px; background-color: rgb(255, 255, 255);\">　　2．5　 建立静脉通道。有颅内高压病人，应快速静脉输入２０％甘露醇或其它脱水剂，注意静脉补液量应适宜，太多导致脑水肿加重，太少可产生水、电解质平衡紊乱。</span><br/><span style=\"font-family: Arial, 宋体; font-size: 14px; line-height: 25.2px; background-color: rgb(255, 255, 255);\">　　2．6 降温。降温以物理降温为主，可采用酒精擦浴、冰袋、冰帽。</span><br/><span style=\"font-family: Arial, 宋体; font-size: 14px; line-height: 25.2px; background-color: rgb(255, 255, 255);\">　　2．7　 脱水和止血治疗。在疑有脑疝形成的病人，应立即给予脱水剂、利尿剂和激素。２０％甘露醇１２５—２５０毫升，快速静脉滴注（３０分钟）６—８小时１次，以降低颅内压，改善脑水肿，脱水剂使用时应依据病情而定，一般２周左右，应经常观察肾功能，认真记录２４小时出入量，定时检查电解质及血气分析，注意水电解质平衡，止血药常用６—氨基己酸６—１２小时１次，静脉滴注。</span><br/><span style=\"font-family: Arial, 宋体; font-size: 14px; line-height: 25.2px; background-color: rgb(255, 255, 255);\">　　2．8 　有条件情况下，可行颅内压监测，以便及时发现颅内压升高。</span><br/><span style=\"font-family: Arial, 宋体; font-size: 14px; line-height: 25.2px; background-color: rgb(255, 255, 255);\">　　2．9　 外科手术治疗。病人年龄在６０岁以下，有明确的颅内血肿形成，在脑疝形成前期或ＣＴ证实血肿直径在３５毫米以上者为外科手术指征。</span><br/><span style=\"font-family: Arial, 宋体; font-size: 14px; line-height: 25.2px; background-color: rgb(255, 255, 255);\">　　1.3 康复训练：在患者生命体征稳定、神经症状不再发展后可进行康复训练，术后康复训练主要包括语言训练、肢体训练以及认知训练，其中语言训练主要通过用读报纸、读杂志、听收音机等方式促进患者语言功能的恢复。肢体训练协助患者由进行体位转换，做单、双杠训练以及排便、翻身等基本训练。认知训练为利用简单的识记读本、进行趣味的记忆答题。</span><br/><span style=\"font-family: Arial, 宋体; font-size: 14px; line-height: 25.2px; background-color: rgb(255, 255, 255);\">　　2 讨论</span><br/><span style=\"font-family: Arial, 宋体; font-size: 14px; line-height: 25.2px; background-color:
