255);\">　脑溢血,又称脑出血,它起病急骤、病情凶险、死亡率非常高,是急性脑血管病中最严重的一种,为目前中老年人致死性疾病之―。一般来说,脑出血一旦确诊,应首先给脱水、降压措施,根据血肿的大小、部位、病人的一般状况及临床表现来适时决定是否进行手术治疗。总的来说,壳核出血大于30ml、丘脑出血大于14ml、小脑半球出血大于15ml、小脑蚓部出血大于6ml;壳核出血发展到内囊后肢破入或不破入脑室,壳核出血发展到内囊的前后肢,丘脑出血量大于15ml,累及丘脑或丘脑下部,破入或不破入脑室;病人处于昏睡、浅昏迷但无脑疝或脑疝早期、意识状态呈进行性加重[1],内科治疗无好转等都应考虑手术治疗。在治疗的过程中,护理工作与医生抢救紧密配合,才能尽可能地降低病死率和残疾率。现将我院33例脑出血患者术后护理体会,总结如下。&nbsp;</span><br/><span style=\"font-family: Arial, 宋体; font-size: 14px; line-height: 25.2px; background-color: rgb(255, 255, 255);\">　　1 资料与方法&nbsp;</span><br/><span style=\"font-family: Arial, 宋体; font-size: 14px; line-height: 25.2px; background-color: rgb(255, 255, 255);\">　　1.1 一般资料本组脑出血病例33例,男19例,女14例,年龄48～70岁。&nbsp;</span><br/><span style=\"font-family: Arial, 宋体; font-size: 14px; line-height: 25.2px; background-color: rgb(255, 255, 255);\">　　1.2 临床症状14例在活动时出现一侧肢体偏瘫,失语,昏迷;深昏迷7例;3例饮酒后出现偏瘫昏迷;另外9例发病情况不详。&nbsp;</span><br/><span style=\"font-family: Arial, 宋体; font-size: 14px; line-height: 25.2px; background-color: rgb(255, 255, 255);\">　　1.3 临床诊断有程度不同的意识障碍,早期多血压显著升高,重症者脉洪缓慢,呼吸深缓,常伴中枢性高热,病情恶化时呈现中枢性呼吸、循环衰竭,瞳孔形状不规则、双侧缩小或散大、双侧大小不等,光反应迟钝或消失。脑膜刺激征阳性,眼底可见视网膜动脉硬化和视网膜出血,偶有视乳头水肿,可有上消化道出血,心律不齐、肺水肿等。出现肢体偏,局限性定位体征诊断。瘫昏迷患者入院时均有不同程度的血压升高,本组33例病人中26例有明确的高血压病史。&nbsp;</span><br/><span style=\"font-family: Arial, 宋体; font-size: 14px; line-height: 25.2px; background-color: rgb(255, 255, 255);\">　　1.4 治疗方法与转归33例患者均采用手术治疗。33例患者中,痊愈4例,显著好转14例,好转10例,死亡5例。死亡原因:出现心功能及心电异常死亡2例,消化道出血死亡1例,肾功能衰竭死亡1例。&nbsp;</span><br/><span style=\"font-family: Arial, 宋体; font-size: 14px; line-height: 25.2px; background-color: rgb(255, 255, 255);\">　　2 术后护理&nbsp;</span><br/><span style=\"font-family: Arial, 宋体; font-size: 14px; line-height: 25.2px; background-color: rgb(255, 255, 255);\">　　2.1 生命体征观察严密观察患者的意识、血压、呼吸、体温、脉搏是否正常,及时做好记录,如有异常,及时通知医生。</span><br/><span style=\"font-family: Arial, 宋体; font-size: 14px; line-height: 25.2px; background-color: rgb(255, 255, 255);\">　　2.2 呼吸护理遵医嘱,药物祛痰,加强翻身、拍背,使痰液松动咳出,减轻肺部感染。无力咳痰者,采取吸痰措施,保持呼吸道通畅[2]。&nbsp;</span><br/><span style=\"font-family: Arial, 宋体; font-size: 14px; line-height: 25.2px; background-color: rgb(255, 255, 255);\">　　2.3 一般护理一个安静、舒适的环境,特别是发病2周内,应尽量减少探望,保持平和、稳定的情绪,避免各种不良情绪影响。头部可轻轻向左右转动,应避免过度搬动或抬高头部,四肢可在床上进行小幅度翻动,每2小时一次,避免瘫痪肢体皮肤长期受压坏死,发生褥疮。大小便须在床上进行,不可自行下床解便,以防再次出血的意外发生。&nbsp;</span><br/><span style=\"font-family: Arial, 宋体; font-size: 14px; line-height: 25.2px; background-color: rgb(255, 255, 255);\">　　2.4 饮食护理术后6h可给予少量温开水喝,无呕吐可少量多餐进食。术后3天未清醒可行插胃管饲流质饮食。通过进食可减轻胃酸对胃粘膜的刺激,减少消化道出血。同时通过对胃液的观观察了解消化道是否有出血及出血量。给予食物要营养丰富、低脂、清淡软食,如鸡蛋、豆制品等。进食困难者,可头偏向一侧,喂食速度慢,避免交谈,防呛咳、窒息。为保持大便通畅,可食用香蕉、蜂蜜,多进水,加强适度翻身,按摩腹部,减少便秘发生。病人数天未解便或排便不畅,可使用缓泄剂,诱导排便。禁忌用力屏气排便,防再次脑出血。&nbsp;</span><br/><span style=\"font-family: Arial, 宋体; font-size: 14px; line-height: 25.2px; background-color: rgb(255, 255, 255);\">　　2.5药物护理:遵医嘱给予药物。服用降压药物要按时定量,不随意增减药量,防血压骤升骤降,加重病情,注意给药顺序。建议患者亲友或家属不要自行再加用许多药物。过多、过乱的应用药物,对胃、肝、肾或造血系统有可能产生副作用,不但不能加快恢复,反而可引出其他问题。&nbsp;</span><br/><span style=\"font-family: Arial, 宋体; font-size: 14px; line-height: 25.2px; background-color: rgb(255, 255, 255);\">　　2.6 心理护理急性期家属及病人的注意力在抢救生命上,而在康复期则往往急于功能恢复,要求很快自理,甚至去工作。要求用新药、新方法治疗者颇多;有部分病人表现悲观、失望,精神抑郁。因此,护士要有耐心,经常用解释,开导,劝慰的语言。要多鼓励病人树立战胜疾病的信心,要实事求是地对待自己的疾病和功能,在治疗护理中给予鼓励调动病人的积极性,积极配合治疗。&nbsp;</span><br/><span
