255);\">　　高血压性脑出血,发病后1周内病死率最高,早期血压能否合理管理是影响病情转归和预后的重要因素之一。关于该病理想血压的控制问题一直存在争论,尚无统一标准。脑出血合并高血压危象是一种高发病率、高致残率和高复发率的临床急症[1]。为此,本文对照分析我院收治的260例高血压性脑出血患者早期血压管理与病情转归的关系,探讨高血压性脑出血早期血压管理问题。&nbsp;</span><br/><span style=\"font-family: Arial, 宋体; font-size: 14px; line-height: 25.2px; background-color: rgb(255, 255, 255);\">　　1 临床资料&nbsp;</span><br/><span style=\"font-family: Arial, 宋体; font-size: 14px; line-height: 25.2px; background-color: rgb(255, 255, 255);\">　　1.1 一般资料 我科自2006 年以来收治的高血压性脑出血患者260例,男172例,女88例。年龄34-88岁,全部病例均有高血压病史,在起病后6小时内入院,头颅CT诊断为基底节区出血,入院时收缩压&gt;180mmHg、舒张压&gt;110mmHg(1mmHg=0.133kPa)。入院后给予20%甘露醇125ml静注,每日3-4次,部分患者给予甘油果糖或七叶皂苷静滴脱水降颅压,部分患者给予20mg速尿静推,对躁动者给予适当镇静。观察血压1-3小时持续不降者,给予降压药物20%硫酸镁10ml深部肌注,如血压仍持续不降,部分患者给予硝普钠静脉泵入，部分患者未再对血压作积极处理。全部患者均给予防治并发症治疗。&nbsp;</span><br/><span style=\"font-family: Arial, 宋体; font-size: 14px; line-height: 25.2px; background-color: rgb(255, 255, 255);\">　　1.2 病例分组 将出血量&lt;30ml和&gt;30ml的患者分为两大组,观察入院后6h、12h、24h、48h、72h的血压情况。按血压控制情况,出血量&lt;30ml的患者分为Ⅰ组(血压控制满意组)和Ⅱ组(控制不满意组)，将出血量&gt;30ml患者,分为Ⅲ组(血压控制满意组)和Ⅳ组(控制不满意组),见表1。Ⅰ组和Ⅱ组、Ⅲ组和Ⅳ组,入院时的平均血压、年龄、出血量、出血部位均具有可比性。&nbsp;</span><br/><span 患者入院后7天作病情评判。好转:患者神经系统症状改善。稳定:患者神经系统症状无恶化表现,无严重并发症。恶化:神经系统症状加重或出现严重并发症、多器官功能衰竭。死亡:剔除24小时内死亡病例。对好转和稳定的病例认为抢救成功,对恶化和死亡的患者判为抢救失败。&nbsp;</span><br/><span style=\"font-family: Arial, 宋体; font-size: 14px; 高血压性脑出血是高血压病伴发的脑小动脉病变在血压骤升时破裂所致。其早期的血压增高,一方面是骤升血压的持续状态,是出血的病因基础,另一方面是血肿的占位效应及血肿周围脑组织水肿致颅内压增高,机体通过血管调节系统使血压反射性增高,以保证脑灌注,乃代偿机制所致。血压持续显著增高,可导致脑血管强制扩张,脑血流量被动增加,加重脑水肿。血压过低,可导致脑灌注压下降,脑灌流不足,加重脑缺血。有研究表明[2]，脑卒中14天内的早期病死率以及远期预后,与患者的血压呈“U ”型曲线关系,当收缩压在140-180mmHg，病死率最低、预后最佳,而血压太低(收缩压&lt;140mmHg)或血压太高(收缩压&gt;180mmHg)患者的预后较差,病死率增加。最新的美国心脏病协会指南指出，脑出血缺乏一个血压的特定值，尤其是急性脑出血，只是认为收缩压超过200mmHg或平均动脉压超过150mmHg时应该积极控制血压[3]。本组资料表明,适当调控高血压性脑出血早期高血压能明显提高该病抢救成功率,降低早期病死率。笔者认为,在调控早期高血压时还应重视以下两点:1)应根据血肿量大小不同采用不同的血压维持脑灌注,血肿量越大、脑水肿越重,则颅内压越高,需要较高的平均动脉压维持脑灌注压。2)应了解早期血压增高的原因,是导致脑出血的骤升血压的持续状态,还是由于机体代偿机制所致的血压增高。如是前者应积极处理,后者则降压宜谨慎。脑出血患者经积极脱水降颅压处理,如血压缓慢下降,则考虑为机体代偿机制所致。如血肿量小而血压显著增高,则考虑为骤升血压的持续状态。高血压性脑出血早期高血压调控,公认的原则是慎重、适度。结合文献报告我们体会:1)把血肿量的大小作为血压调控的参考。血肿量&lt;30ml者、收缩压&gt;180mmHg、舒张压&gt;110mmHg,血肿量&gt;30ml者、收缩压&gt;200mmHg、舒张压&gt;110mmHg,通过脱水降颅压等治疗,血压持续不降,应给予降血压药物治疗。同时应对患者发病后的精神紧张、持续疼痛、躁动、睡眠障碍等可加重高血压的因素积极处理。2)降压速度不能过快,幅度不能过大。血肿量&lt;30ml者,24小时血压控制在150-170mmHg,48-72小时控制在140-160mmHg。血肿量&gt;30ml者,24小时血压逐渐控制在170-190mmHg、48-72小时控制在150-170mmHg。有研究表明[1],头24小时使平均血压降低约10%-20%为宜。3)结合掌握血压逐渐降至并维持在比病人发病前的基础血压高10-20mmHg水平。4)合理选择降压药物。大部分病人经肌注硫酸镁能达到降压效果;无效者,可选用硝普钠静脉泵入。硝普钠具有起效快半、衰期短,极易计量的优点,理论上可增高颅内压,然而临床应用方面未出现潜在的不良反应。临床实践表明，在脑出血的预防和治疗中，血压调控是非常重要的有效手段，应引起足够的重视[4]。血压满意控制后,对意识清醒能口服者,最好选用长效降压药物,以免24小时血压波动过大。选用口服降压药物应遵循个体化原则,避免降压过度。&nbsp;</span></p>","barId":"1854","topicTypeId":null,"topicThemeId":"1029","readLevel":0,"files":"","clickNoLikeCount":0,"clickLikeCount":0,"clickCount":0,"score":0,"partCount":0,"replayCount":0,"publishStatus":0,"publishTime":0,"isCream":null,"canReply":0,"shareUrl":null,"foodCouponVal":0,"emailNotice":0,"permission":0,"zlyShare":0,"status":0,"sort":0,"highlight":0,"highlightEffectTime":0,"digest":0,"digestEffectTime":0,"top":0,"topEffectTime":0,"closed":0,"stamp":0,"icon":0,"bgcolor":null,"fontcolor":null,"favtimes":0,"isAdopt":0,"adoptId":null,"sortDate":0,"metaSearch":null,"rn":1},{"id":"22621","isDel":0,"createAt":0,"createBy":null,"createName":null,"updateAt":0,"updateBy":null,"updateName":null,"userId":null,"title":"脑出血能好吗","content":"<p style=\"font-family:宋体\">防中风当心中午</span>11点。缺血性中风发病高峰时间为11时32分左右，出血性中风发病高峰时间为12时11分左右。老年人应该牢记这两个时间，睡午觉时如果突然觉得翻身困难，动弹不了，应马上呼救。</span></p><p
